- Initial overall evaluation by Physician Health Advisor who is a locally registered GP
- Goal setting and development of a personalized action plan
- Ongoing support from a multidisciplinary team, including a dietitian if needed
- Continuous monitoring and timely adjustments to the action plan by the Physician Health Advisor based on any changes in the condition
- Regular follow-ups conducted by a Registered Nurse
- Detailed interim summary report provided after 2 months
- Detailed final summary report provided after 12 months
- An active PRUHealth Wherever You Go insurance plan;
- A prior diagnosis of one of these conditions: hypertension, prediabetes, diabetes, or hyperlipidemia; and
- Previous medical inquiries and consultations with at least one medical practitioner.

- Fasting glucose 6.1 – 6.9 mmol/L
- HbA₁c 5.7 – 6.4%
- Fasting glucose ≥ 7.0 mmol/L
- HbA₁c ≥ 6.5%
- Total cholesterol > 5.2 mmol/L
- LDL cholesterol ≥ 3.4 mmol/L
- HDL cholesterol < 1.0 mmol/L
- Triglycerides ≥ 1.7 mmol/L
- A prior diagnosis of one of these conditions: hypertension, prediabetes, diabetes, or hyperlipidemia; and

The Chronic Care Management Programme offers one-to-one, personalised and tailored care program based on the customer’s health conditions. It is coached by a dedicated Physician Health Advisor, Registered Nurse and a dietician to intervene health conditions early and avoid further complications.
The covered conditions include hypertension, prediabetes, diabetes, and hyperlipidemia.
Your Physician Health Advisor will reach out by phone or email to coach and support lifestyle changes that can improve your health.
For the avoidance of doubt, the service is not meant to provide you with healthcare and/or timely/ongoing medical treatment nor to create a doctor-patient relationship, but to guide you in changing your lifestyle to improve your health conditions.
This programme is designed to support you in making lasting lifestyle changes over 12 months, including weekly follow-ups for the first 2 months, followed by monthly follow-ups for another 10 months.

Please note that the result of the review is based on the latest information provided on your condition and the decision is at the discretion of Prudential. Prudential reserves the right to ask for any further medical evidence or examination as proof of the condition.
